Scarce vintage Theatre World Magazine request form for biographical information on an 8.5 x 11-inch sheet of tan paper, completely filled out by hand and boldly signed in fountain pen, circa 1947.  In good condition for its age, with gentle toning, scattered surface creases and multiple mailing folds.  The veteran English character actor of the 1930s through 1970s was memorable in Hammer Studios' Horror of Dracula (1958) and The Creeping Flesh (1973), both with Peter Cushing and Christopher Lee.  His other contributions to the horror, sci-fi and fantasy genres include Helter Skelter (1949), The Man in the White Suit (1951), Vampire Over London (1952), Three's Company (1954), Journey Into Darkness (1968), What Became of Jack and Jill (1972), and A Warning to the Curious (1972).
